About The Position

FOX Rehabilitation is seeking licensed Physical Therapists in Goshen, New York, to provide one-on-one care to older adults in private residences and senior living communities. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services where appropriate. This role offers autonomy to create, manage, and adjust a treatment schedule, build lasting patient relationships, and work across in-home and senior living settings. Clinicians practice in a collaborative, interdisciplinary environment and are supported by a clinician-operated organization focused on evidence-based care, meaningful relationships, and abolishing ageism. Full-time, part-time, and PRN/Fox Flex employment pathways are available. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. Compensation is based on experience and employment type. The stated New York standard base pay range for full-time roles is $75,000–$115,000 annually, with the opportunity to earn more; part-time and FOX Flex compensation is prorated. Eligible roles may include medical, dental, and vision insurance, a 401(k) with company match, continuing education access, and professional certification reimbursement. Clinicians may also earn additional compensation through referral programs, teaching, mentoring, supervising assistants, exceeding patient-care quotas, and creating content.

Requirements

Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including exerting medium physical effort; lifting 25–50 pounds occasionally, 10–25 pounds frequently, and up to 10 pounds constantly; performing CPR; providing moderate to maximum assistance with patient transfers and bed mobility; and standing, walking, climbing stairs, and performing therapeutic activities for extended periods.
Ability to drive to patient homes and senior living communities.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
Perform patient transfers and bed mobility assistance, reposition patients, and conduct therapeutic activities.
Wear PPE when required.
