About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in New York, NY and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also incorporate telehealth and other virtual care into their schedules.

FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. The practice offers full-time, part-time, and PRN/FOX Flex employment pathways. New graduates may be eligible for the Emerging Professionals Mentor Program. Benefits are available for eligible roles and include medical, dental, and vision insurance, a 401(k) with company match, continuing education access, and professional certification reimbursement. In New York, the standard annual base pay range for a full-time role is $75,000–$115,000, with an opportunity to earn more; part-time and FOX Flex compensation is prorated.

Requirements

• Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, stair climbing, and therapeutic activities.
• Ability to drive to patient homes and senior living communities.
• Wear personal protective equipment when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
• Potentially supervise PTAs or OTAs, mentor emerging professionals, or serve as a Clinical Instructor, as applicable.
