About The Position

FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in private residences and Senior Living Communities in Sutton Place, New York. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, longevity, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also provide telehealth and other virtual care when appropriate. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. The position involves working in a collaborative, interdisciplinary environment and may include in-home, senior living, and virtual care. Full-time, part-time, and PRN / FOX Flex employment options are available. New graduates are eligible for the Emerging Professionals Mentor Program. In New York, the standard base pay range for a full-time role is $70,000–$105,000 annually, with an opportunity to earn more. Part-time and FOX Flex (PRN) compensation will be prorated.

Requirements

Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Occupational Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one occupational therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
