About The Position

Flagstar Rehab is seeking Physical Therapists to provide therapy services in skilled nursing facilities in Southampton, New York. The role involves evaluating patients, developing individualized treatment plans for physical therapist assistants to follow under the therapist’s supervision, and delivering therapy to individuals with conditions including traumatic brain injury, CVA, stroke, Parkinson’s disease, multiple sclerosis, cancer, and orthopedic surgery. Therapists monitor and document patient progress using EMR software. The employer describes opportunities for structured schedules or per diem work and offers support with placement and communication. Independent contractors may receive guidance and LLC support. Compensation is listed as $64 per hour for 1099 work. Available schedules include full-time, part-time, per diem, weekend, day, evening, and coverage shifts. Independent contractors are not eligible for all benefits.

Requirements

Current New York State Physical Therapist license and registration.
Valid BLS certification.
EMR competence.
Meets all state and federal requirements for employment in a U.S. nursing facility.
Malpractice/liability insurance, if applicable.

Responsibilities

Provide efficient patient evaluations.

Provide therapy services to skilled nursing facility patients requiring care related to TBI, CVA, stroke, Parkinson’s disease, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
Monitor and document patient progress using EMR software.
