About The Position

The Physical Therapist delivers high-quality, safe home physical therapy in accordance with federal, state, professional, and agency standards and requirements. The position provides population-appropriate therapeutic interventions, monitors patient progress and response to treatment, and supports patients and families in maintaining functional independence. Travel is always required. Other duties may be assigned as necessary.

Requirements

Graduate of a recognized school of physical therapy with a Bachelor of Science degree.
Current licensure in the state of primary practice.

Responsibilities

Deliver high-quality, safe home physical therapy in accordance with applicable federal, state, professional, and agency standards and requirements.
Use independent judgment and strong planning and problem-solving skills.
Communicate and demonstrate a professional image and attitude to patients, families, clients, coworkers, and others.
Provide population-appropriate therapeutic interventions to achieve positive outcomes as assessed through quality monitoring and patient satisfaction.
Supervise Home Health Aides or Physical Therapy Assistants and document supervision appropriately.
Document patient-care progress, continuing treatment needs, and responses to treatment.
Educate patients and families about tasks and resources that support continued functional independence.
Participate in continuing education and professional development activities.
Participate in performance improvement management activities and committees sponsored by the agency and Guthrie Healthcare System.
Contribute to cost-effective care while meeting visit and productivity requirements stipulated by agency administration.
Travel as required for the position.
