About The Position
This is a travel assignment for a Physical Therapist (General) in the White River Junction, VT area. The contract is for 16 weeks, with guaranteed 32 hours per week during the day shift. The setting is home healthcare, specifically hospice care. The role involves screening, evaluating, and treating patients in their homes, developing individualized treatment plans, coordinating with the interdisciplinary care team, documenting patient progress, and educating patients and their families.
Requirements
- Bachelor of Science in Physical Therapy
- Licensed Physical Therapist in VT and NH, or hold an unencumbered compact license
- Minimum 1 year of PT experience in a home healthcare setting
- Ability to work effectively in a home-based care environment
Responsibilities
- Screen, evaluate, re-evaluate, and treat patients in a home health setting
- Develop and implement individualized physical therapy treatment plans in line with agency policies and professional standards
- Coordinate and communicate treatment activities with the interdisciplinary care team and patients
- Document evaluations, treatment goals, progress, and outcomes
- Educate patients and family members on the rationale for treatments and procedures
