About The Position

Sage Group HealthCare Staffing is seeking a Physical Therapist for a 13-week travel/contract position in Marion, OH. This role involves providing in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability. The goal is to allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home. The therapist will consult with physicians and an interdisciplinary team to develop treatment plans and will utilize skills in patient evaluation and treatment to achieve maximum patient independence. Daily travel to patient homes in the assigned territories (Marion, OH and surrounding areas) is required.

Requirements

• Current Physical Therapist experience: 1-2 years - Required
• Recent/Current Home Health PT experience: 12 months within the last 3 years - Required
• Home Care Home Base (HCHB) charting experience - Required
• OASIS experience - Required
• Must have valid Driver’s License and Personal Auto Insurance
• Bachelor of Science degree in Physical Therapy or Similar – Required
• Current State License as Physical Therapist or Compact if applicable - Required
• BLS Certification from AHA or American Red Cross - Required

Responsibilities

• Provide Physical Therapy services to patients/clients at their homes.
• Consult with the physician and an interdisciplinary team in the development of a plan of treatment.
• Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.
• Plans and prepares treatment plan based on evaluation of patient data.
• Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability.
• Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home.
• Drive to patient homes in the assigned territories on daily basis.
