About The Position
FOX Rehabilitation is seeking a licensed Physical Therapist Assistant to provide hands-on care to older adults in Marlboro, New Jersey, and surrounding communities. The role follows FOX’s Geriatric House Calls™ model, delivering individualized care in patients’ homes and senior living communities. The PTA works closely with a supervising Physical Therapist to carry out treatment plans, guide patients through exercises and functional activities, and help them improve strength, mobility, confidence, and independence. This position offers one-on-one patient care, collaboration with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists, and mentorship from a supervising PT. Requirements
- Valid Physical Therapist Assistant license in the state or states of practice, or eligibility to obtain one.
- Graduate of an accredited Physical Therapist Assistant program.
- Current CPR certification.
- Basic computer skills and proficiency in Microsoft Office.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, extensive walking, and wearing PPE when required.
- Ability to drive to patient homes and senior living communities.
Responsibilities
- Deliver individualized treatment plans established by the supervising Physical Therapist, adjusting activities based on patient response and progress.
- Instruct, motivate, and safeguard patients during therapeutic exercises and functional activities, while observing and tracking their progress.
- Document visits accurately and on time in FOX’s EMR system according to FOX’s Clinical Documentation standards.
- Stay connected with patients, powers of attorney, and community referral sources, and help coordinate coverage when needed.
- Provide in-home and community-based care, including driving to patient homes and senior living communities.
- Work collaboratively with the interdisciplinary care team and supervising Physical Therapist.
