About The Position

FOX Rehabilitation is seeking licensed Physical Therapist Assistants in Monroe, New Jersey, to provide hands-on care to older adults through its Geriatric House Calls™ model. Care is delivered in patients’ homes and senior living communities. The PTA works closely with a supervising Physical Therapist to carry out individualized treatment plans and help patients rebuild strength, mobility, confidence, and independence. The role offers one-on-one patient care in a supportive, team-based environment. PTAs collaborate with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and Fitness Specialists, with mentorship from the supervising PT and opportunities to develop clinical skills, caseload, and career. Full-time, part-time, and PRN/FOX Flex employment pathways are available. Compensation is competitive based on experience and employment type. The standard base pay range for this role in New Jersey is $24.04–$45.00 per hour, with opportunities to earn more.

Requirements

Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain one.
Graduate of an accredited Physical Therapist Assistant program.
Current CPR certification.
Basic computer skills and proficiency in Microsoft Office.
Ability to meet the physical demands of in-home and community-based care, including specified lifting, patient assistance, mobility, and movement requirements.

Responsibilities

Deliver individualized treatment plans established by the supervising Physical Therapist, adjusting activities based on patient response and progress.
Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities, while observing and tracking progress.
Document visits accurately and on time in FOX’s EMR system, in accordance with FOX’s Clinical Documentation standards.
Stay connected with patients, POAs, and community referral sources, and help coordinate coverage when needed.
Work collaboratively with the interdisciplinary care team, including PTs, OTs, SLPs, and Fitness Specialists.
Drive to patient homes and senior living communities to provide care.
