About The Position

Adaptive Home Health is building a higher-acuity, patient-centered skilled home health model across Texas to improve patient access to home health care. The Physical Therapist Assistant (PTA) is a core part of this mission, helping restore function, keep patients safe at home, and drive patient outcomes in partnership with a supervising PT. The company provides technology to support field clinicians, including a charting product designed to reduce documentation burden and a scheduling product planned to streamline routing and visit coordination. The PTA delivers skilled visits in patients’ homes under the supervising PT’s plan of care. The role focuses on functional mobility and measurable progress, including gait and balance, transfers, strengthening, home safety, and fall prevention. The PTA tracks objective progress within PTA scope, communicates updates and concerns to the supervising PT, coordinates with the care team, and coaches patients and caregivers on practical home programs. Documentation is completed promptly and thoroughly in Homecare Homebase (HCHB). This is a full-time, field-based position with a Monday–Friday schedule and occasional weekends as needed. Compensation is $58 per productivity point, with stated annual compensation of $75,000–$100,000.
