About The Position

Ultimate Therapy is hiring a full-time Physical Therapist Assistant at Autumn Lake at Voorhees West in Voorhees, New Jersey. The role supports residents’ recovery in a skilled nursing rehabilitation setting through treatment focused on practical functional goals, such as standing more steadily and walking with less assistance. The assistant provides treatments under the supervising Physical Therapist’s plan of care and collaborates with nursing and rehabilitation staff to support residents’ mobility in daily care.

Requirements

• Active New Jersey Physical Therapist Assistant license.
• Ability to follow individualized treatment plans and recognize changes requiring the Physical Therapist’s attention.
• Strong communication, teamwork, and clinical documentation skills.

Nice To Haves

• Skilled nursing experience is helpful; new graduates are welcome to apply.

Responsibilities

• Provide physical therapy treatments outlined in the supervising Physical Therapist’s plan of care.
• Use prescribed exercises and functional activities to build strength, balance, and endurance.
• Guide residents through transfer training, walking practice, and bed mobility while observing individual precautions.
• Reinforce safe use of walkers and other prescribed equipment during daily movement.
• Track treatment tolerance and functional changes, and share observations with the supervising Physical Therapist.
• Maintain accurate session notes.
• Collaborate with nursing and rehabilitation staff to carry mobility strategies into daily care.
