In this Skilled Nursing Facility (SNF) role, the Physical Therapist Assistant treats patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning. The position involves developing individualized treatment plans and collaborating with nursing staff, physicians, families, and the wider interdisciplinary care team to support patients in reaching their highest level of independence. The company describes its SNF teams as collaborative and its care as evidence-based, with reasonable productivity expectations and respect for clinical judgment. Requirements

Associate's degree from an accredited PT program.
Current or eligible for a Connecticut State PTA license.
Passion for geriatric rehabilitation and functional outcomes.
Strong communication and teamwork skills.
