About The Position

Physicians Alliance of Connecticut (PACT, LLC), a multi-specialty physician group in partnership with Hartford Healthcare, is seeking a Physiatry Physician to work with their PACT Primary Care West Haven offices. The office will be located in Hartford. This role involves providing and managing health care for a specified patient population, including direct patient care, physical examinations, evaluations, assessments, diagnoses, and treatment. The physician will engage patients and their families in understanding and monitoring self-management care plans, documenting individualized care plans and care coordination. The role also includes analyzing medical history, diagnosing conditions, prescribing medications and treatments, referring patients to specialists, respecting patient rights, responding to emergencies, and participating in practice process evaluations, quality improvement efforts, health promotion, education, prevention programs, and population health activities.

Requirements

  • Medical Doctor with 3 to 5 years of direct patient experience which may include residency.
  • Medical License
  • Board certification
  • Federal DEA certification
  • Board Certified Physiatrist
  • Knowledge of legal and ethical standards for the delivery of medical care.
  • Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in Physiatry.
  • Demonstrated skill in interviewing and patient coaching.
  • EHR documentation as it pertains to patients and populations of patients.

Responsibilities

  • Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
  • Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
  • Analyze patients medical history, medication allergies, physical condition and examination to determine best care
  • Diagnose medical condition
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Refers patients to specialists or to internal medicine as appropriate.
  • Respect and promote patient rights and privacy.
  • Responding appropriately to emergency situations.
  • Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
  • Participate in specified health promotion, education and/or prevention programs.
  • Participates in population health activities as directed by the practice.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
