About The Position

Physicians Alliance of Connecticut (PACT, LLC), in partnership with Hartford Healthcare, is seeking two Physicians to work with their PACT Primary Care West Haven offices, located in Milford. Under indirect supervision, the physician will provide and manage healthcare for a specified patient population. This includes conducting physical examinations, evaluations, assessments, diagnoses, and treatments. The role involves engaging patients and their families in understanding and monitoring self-management care plans, documenting individualized care plans, and coordinating care. The physician will analyze patient medical history, allergies, and physical condition to determine the best course of care, diagnose medical conditions, and prescribe appropriate medications and treatment regimens. They will also refer patients to specialists or internal medicine as needed, direct and coordinate patient care activities of nursing and support staff, and respect patient rights and privacy. Additionally, the physician will respond to emergencies, participate in meetings to evaluate practice processes and quality improvement efforts, and engage in health promotion, education, prevention, and population health activities as directed. Adherence to established departmental policies, procedures, objectives, and quality improvement standards is expected.

Requirements

  • Medical Doctor with 3 to 5 years of direct patient experience which may include residency.
  • Medical License
  • Board certification
  • Federal DEA certification
  • Knowledge of legal and ethical standards for the delivery of medical care.
  • Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise.
  • Demonstrated skill in interviewing and patient coaching.
  • EHR documentation as it pertains to patients and populations of patients.

Responsibilities

  • Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
  • Engages patients and their families or caregivers in understanding, setting and monitoring self management care plans, and documents individualized care plans and care coordination in the patient chart.

  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Refers patients to specialists or to internal medicine as appropriate.

  • Respect and promote patient rights and privacy.
  • Responding appropriately to emergency situations.
  • Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
  • Participate in specified health promotion, education and/or prevention programs.
  • Participates in population health activities as directed by the practice.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
