About The Position

Physicians Alliance of Connecticut (PACT, LLC), in partnership with Hartford Healthcare, is seeking two Physicians to work with their PACT Primary Care West Haven offices. The role involves providing and managing comprehensive healthcare for a specified patient population, including physical examinations, evaluations, assessments, diagnoses, and treatment. The physician will also engage patients and their families in self-management care plans, diagnose medical conditions, prescribe medications and treatments, refer patients to specialists, and direct patient care activities of support staff. The position emphasizes respecting patient rights and privacy, responding to emergencies, and participating in quality improvement, health promotion, and population health activities. Adherence to established departmental policies, procedures, and safety standards is required.

Requirements

  • Knowledge of legal and ethical standards for the delivery of medical care.
  • Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise.
  • Demonstrated skill in interviewing and patient coaching.
  • EHR documentation as it pertains to patients and populations of patients.
  • Medical Doctor with 3 to 5 years of direct patient experience which may include residency.
  • Medical License
  • Board certification
  • Federal DEA certification

Responsibilities

  • Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
  • Engages patients and their families or caregivers in understanding, setting and monitoring self management care plans, and documents individualized care plans and care coordination in the patient chart.
  • Analyze patients medical history, medication allergies, physical condition and examination to determine best care
  • Diagnose medical condition
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Refers patients to specialists or to internal medicine as appropriate.
  • Directs and coordinates the patient care activities of nursing and support staff as required.
  • Respect and promote patient rights and privacy.
  • Responding appropriately to emergency situations.
  • Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
  • Participate in specified health promotion, education and/or prevention programs.
  • Participates in population health activities as directed by the practice.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
