About The Position

The Program Specialist provides support for the nurse home visiting, case management, and care coordination services including Babies First, CaCoon, and Family Connects Oregon. Family Connects is an evidence-based universally offered nurse home visiting program that connects parents/caregivers of newborns to the community resources they need after birth. The model was developed in 2008 through a partnership with Duke University, the Center for Child & Family Health (CCFH), and the Durham County Health Department. Perinatal, Maternal, Child & Family Health programs are community-based public health services that support people who are pregnant, new parents/caregivers, newborns, infants, children up to age 5, and children and youth through 21 with special health needs. Each program offers physical assessments of the client/family, addresses questions about caring for themselves and their children, identifies needs, and helps to identify community services or resources that can meet those needs. The Program Specialist works with a team of nurses to assist in coordinating program referrals, including scheduling home, office, or telehealth visits for people/families in the hospital or clinic, making phone calls, scheduling and coordinating visits and assignments for the nurses, fielding public information lines to answer questions about the program and conduct client experience surveys.

Requirements

Up to one year of technical training beyond high school; and up to 1 year of experience related to area of assignment.
Bilingual Spanish-English required.
Hiring is contingent upon the successful completion of a background check.
This position requires driving and may require use of a personal vehicle.

Responsibilities

Develop and disseminate outreach and other health promotion strategies and materials to engage identified individuals and/or communities.
Develop and maintain collaborative relationships with program partners including, but not restricted to: health care organizations, community-based organizations, other government programs or jurisdictions, businesses, communities of faith, etc.
Participate in planning and conduct outreach at key community events, cultural celebrations, and other venues that provide outreach opportunities among the identified population.
Serve on community coalitions, advisory groups, and task forces serving identified population to increase culturally appropriate outreach, operational coordination/collaboration, and effectiveness.
Serve as a member of an identified project and/or inter-disciplinary team, committee, coalition, etc. working to provide demographically appropriate outreach to identified populations.
Facilitate enrollment and participation in medical and social assistance programs, such as Oregon Health Plan, WIC, and other community-based programs.
Improve and facilitate access to and utilization of internal and external services, resources, and programs.
Review and utilize program-relevant data systems to assist in service delivery, monitor health status indicator measures, and program outcome compliance, improvement and evaluation.
Complete or contribute to program reports as required and assigned.
Maintain compliance with Federal, State, County and other program elements, requirements, deliverables, etc.
Coordinate with graduate and undergraduate student interns and/or program volunteers to support program activities.
Participate in authoring journal articles, media releases, attend professional conferences as a presenter and/or facilitator, and provide training and technical assistance to other groups and organizations.
Participate as an active member of inter-disciplinary teams, and other health services & county initiatives to improve and integrate services.

Annual salary range of $46,463.04 - $66,684.41
This position guarantees two salary raises within your first year .
Increases are given at the end of the 6-month probationary period and at the 1-year anniversary, as long as your performance is in good standing!
Increases are given until the maximum of the salary range is reached.
