About The Position
Angels of Care Pediatric Home Health is hiring Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) for its Pediatric Transitional Nurse Team. The role supports medically fragile children and their families during critical transitions to home care. Nurses provide high-acuity pediatric care and help families maintain safe continuity of care in the home environment through caregiver training and transition support.
Requirements
- Hold an active Texas Registered Nurse or Licensed Vocational Nurse/Licensed Practical Nurse license.
- Have at least one year post-license certification.
- Have current CPR certification.
- Be able to lift and assist pediatric patients as needed.
- Have at least three years of post-graduation experience with tracheostomy care, ventilator management, and TPN.
- Have emergency response management experience.
- Have pediatric home health experience.
- Have experience caring for high-acuity pediatric patients.
Nice To Haves
- Bilingual ability is preferred but not required.
Responsibilities
- Provide skilled, one-to-one pediatric nursing care in patients’ homes.
- Perform patient assessments and ongoing evaluations.
- Administer medications, treatments, and therapies.
- Support hospital-to-home transitions for high-acuity patients.
- Educate and support parents, families, and caregivers.
- Assist with discharge support and care coordination.
- Train and support permanent field nurses during transitions.
- Provide first-shift and interim coverage as needed.
- Collaborate with Clinical Care Managers and interdisciplinary teams.
- Respond appropriately to emergencies and changes in patient condition.
