About The Position

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide dedicated care to a 9-month-old baby girl in a home health setting. The patient has a complex medical history including Monoallelic Mutation of TTN gene, OSA, Hypoaremia, Chronic airway obstruction, PDA, and a Benign tumor of Brachial cleft or vestiges. She requires CPAP at night and has a GJ tube. The LVN will be responsible for all activities of daily living (ADLs), administering medications and feeds via GJ tube, providing GJ tube related care, and all ordered pulmonary care.

Requirements

• LVN license in the state of California
• Proficiency in basic life support and patient care
• Ability to assess and provide nursing care to pediatric patients
• Knowledge of HIPAA regulations and patient privacy
• Willingness to work in home health settings
• Female nurse

Responsibilities

• Provide compassionate and skilled nursing care to pediatric patients in a home setting, ensuring adherence to HIPAA regulations.
• Conduct patient assessments and monitor vital signs, administering basic life support as needed.
• Manage tube feeding, and medication administration, while maintaining accurate electronic health records.
• Collaborate with families and healthcare teams to develop and implement individualized care plans.
• Educate patients and caregivers on health management and safety practices.
• Providing all ADLs
• Administering medications and feeds via gtube
• Providing all gjtube related care
• Providing all pulmonary care as ordered
