About The Position

The Division of Comprehensive Care at Helen DeVos Children's Hospital (HDVCH) is seeking a Pediatric Complex Care APP to join a growing program dedicated to caring for children with medical complexities. This role involves providing comprehensive care primarily in an outpatient setting, with opportunities for inpatient consultations. The APP will manage common clinical issues, conduct perioperative assessments, develop safety and emergency plans, and oversee enteral nutrition plans for dependent children. A key aspect of this position is performing effective, team-based, patient-and family-centered care coordination, advocating for children with medical complexity and their families. The role also includes participating in on-call triage for off-hours patient and family needs, contributing to policy and procedure development related to medical practice and quality patient care, and engaging in process improvement activities. Collaboration with pediatric subspecialists, primary care providers, and the multidisciplinary complex care team, including nurse navigators, is essential for providing a comprehensive overview and facilitating communication.

Requirements

  • Nurse Practitioner: Required master’s degree
  • Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master’s, post-master’s or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
  • 3 years of relevant experience
  • current, relevant clinical experience
  • Previous experience functioning in a collaborative role as a Nurse Practitioner
  • LIC-Nurse Practitioner (NP) - State of Michigan
  • CRT-Basic Life Support (BLS)
  • Physician Assistant: Required master’s degree
  • Graduate of an accredited Physician Assistant educational program
  • LIC-Physician Assistant - State of Michigan
  • CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants
  • CRT-Basic Life Support (BLS)

Responsibilities

  • Providing care primarily in the outpatient setting with inpatient consultation
  • Evaluation and management of common clinical issues in children with medical complexity
  • Provide comprehensive perioperative assessment and management
  • Develop and implement safety and emergency plans
  • Develop and manage enteral nutrition plans for children with enteral nutrition dependence
  • Perform effective, team-based, patient-and family centered care coordination
  • Advocate for children with medical complexity and their families
  • Participate in on call triage off hours for patients and families
  • Provides input into the development of policies and procedures as they related to medical practice and quality patient care
  • Participates in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities
  • Collaborate closely with pediatric subspecialists and primary care providers to provide a comprehensive overview and facilitate communication between teams as needed
  • Collaboration will also occur with the multidisciplinary team within the complex care program including nurse navigators, who provide care coordination to a cohort of medically complex patients
