Patient Service Coordinator

About The Position

KidsCare Home Health is seeking a Patient Service Coordinator to work with the Physician Liaison RN team on non-clinical administrative duties and help maintain a timely departmental workflow. The coordinator builds and maintains relationships with referral sources and internal team members and communicates frequently to follow up diligently on outstanding requests. The role represents KidsCare professionally when working with physician offices.

Requirements

Zero to one year of administrative experience.
High school diploma or GED, or a combination of education and one year of experience.
Ability to read and write simple correspondence and speak effectively in small groups; more than one language may be required.
Basic arithmetic ability, including working with whole numbers, decimals, fractions, and units of measure.
Ability to interpret instructions and solve practical problems involving a variety of concrete variables.
Intermediate computer skills, including Microsoft Office Suite and database activity.
Ability to handle multiple tasks simultaneously.
Experience with specific data-entry systems.

Responsibilities

Maintain open lines of communication with Physician Liaison RNs, therapists, and physician offices.
Represent KidsCare Home Health professionally to physician offices.
Contact offices to verify and confirm receipt of new or outstanding orders and paperwork, and obtain status updates.
Verify client, primary care physician, and well-check information as needed.
Gather outstanding paperwork for the Community Relations team to deliver and retrieve signatures on orders.
Follow up on outstanding orders with physician offices.
Attend team meetings as needed.
Comply with safety policies, practices, and procedures, and report unsafe activity to a supervisor.
Participate in proactive team efforts to achieve departmental and company goals.
Assist with other duties as assigned.
