About The Position

Under the supervision of a registered nurse, the Patient Safety Monitor provides direct or rounding observation of patients identified as a safety risk to themselves and/or others. The role supports patient safety through continuous observation and documentation, communication with the RN and care team, and assistance in maintaining a safe and clean environment. The monitor may assist with activities of daily living when appropriate and accompany patients to medical procedures when continuous 1:1 monitoring is required. The position requires compliance with policies governing patient safety sitters, including suicide and restraint policies; protection of patient rights and confidential health information; professional communication; and prompt reporting of changes in patient behavior or appearance. The employee seeks help from nurses and supervisors when situations are unfamiliar and participates in transfers of care between sitters, nurses, and other care-team members. Requirements

High school diploma or equivalent.
Basic Life Support/CPR certification within 30 days of employment.
Successful completion of the orientation program.
Ability to work cooperatively and collaboratively with others; reliability and flexibility.
Ability to work with and care for a diverse patient population.

Responsibilities

Comply with policies applicable to the patient safety sitter role, including suicide and restraint policies.
Demonstrate ethical judgment and accountability to support patient safety.
Participate in transfers of care between patient safety sitters, nurses, and other care-team members, communicating relevant considerations, priorities, and support needs.
Maintain patient rights and protect the privacy and security of confidential and protected health information.
Help provide a safe and clean environment and assist with activities of daily living when appropriate.
Provide continuous observation and documentation for patients at risk for self-harm, elopement, or injury, as delegated by the RN.
Accompany patients to medical procedures to maintain required 1:1 continuous monitoring.
Observe and report unusual conditions or changes in patient behavior or appearance to the RN.
Complete safety checklists as appropriate and help maintain a safe environment for patients on continuous observation.
Communicate professionally with patients, visitors, and staff.
Use proper administrative and communication procedures to report changes in patient behavior.
Seek assistance from nurses and supervisors in unfamiliar situations.
