Navigator - Memory Clinic - Full Time

About The Position

Under the supervision of the Clinic Manager, the Patient Navigator develops a supportive rapport with patients and caregivers and helps them access health care and community resources and navigate the healthcare continuum. As part of the patient’s collaborative care team, the position coordinates care and addresses care-plan preventive health and wellness goals. The Navigator works with patients to foster self-management and adherence to their clinical plan of care, and collaborates with physicians, the hospital, and community resources to support that plan.

Requirements

Bachelor’s degree in public health or a related health service field.
Knowledge of the healthcare system and available community resources.
Experience with data collection and reporting.
General computer skills, including use of basic computer applications.
BLS certification for healthcare providers, or obtain certification within three months of hire.

Responsibilities

Build supportive relationships with patients and their caregivers.
Help patients and caregivers access health care and community resources and navigate the healthcare continuum.
Coordinate care as a member of the patient’s collaborative care team.
Address preventive health and wellness goals in the care plan.
Work collaboratively with patients to foster self-management and adherence to their clinical plan of care.
Collaborate with physicians, the hospital, and community resources to support patients’ clinical plans of care.
Provide patient education within Broadlawns Medical Center’s sanctioned parameters, using appropriate goals, methodologies, and adult-learning concepts and techniques.
Use sound judgment, effective organizational and communication skills, and appropriate lines of authority.
Maintain effective working relationships with healthcare team members and clients.
Complete annual mandatory education and training within the defined timeframe.
Participate in continuing education and professional development.
