About The Position

Physician Affiliate Group of New York (PAGNY) is seeking a Patient Navigator - Emergency Medicine to join their team at NYC Health + Hospitals/Jacobi and North Central Bronx. This role is part of a multi-faceted, CDC-funded program focused on reducing HIV and other communicable diseases. The position involves case management, planning and conducting disease control efforts, and data analysis for quality improvement under physician supervision. Key responsibilities include identifying candidates for HIV testing in the Emergency Department, performing and documenting rapid HIV testing with pre- and post-counseling, linking newly diagnosed HIV+ patients to care, and connecting eligible patients to appropriate outpatient care (including those with STDs, and those seeking PrEP or PEP follow-up). The role also includes providing counseling and education on PrEP/PEP for HIV- clients, documenting patient interactions in the EMR, participating in focus groups for program enhancement, and attending job-related training.

Requirements

  • High school diploma or equivalent.
  • Experience in case management or patient navigation.
  • Knowledge of HIV testing, counseling, and linkage to care.
  • Familiarity with PrEP and PEP protocols.
  • Proficiency in electronic medical record (EMR) documentation.
  • Ability to conduct rapid, point-of-care HIV testing.
  • Strong communication and interpersonal skills.
  • Understanding of hospital, local, and state health requirements and guidelines.

Nice To Haves

  • Associate's or Bachelor's degree in a related field.
  • Bilingual abilities (English/Spanish).
  • Experience working in an emergency medicine setting.

Responsibilities

  • Identify candidates for routine HIV testing in the adult Emergency Department.
  • Perform and document rapid, point-of-care HIV testing in accordance with hospital, local, and state requirements and guidelines.
  • Provide appropriate pre- and post-counseling and education to clients receiving rapid, point-of-care tests.
  • Link newly diagnosed HIV+ patients to care in the Jacobi or North Central Bronx HIV clinics for immediate antiviral treatment.
  • Link eligible patients to appropriate clinical outpatient care, including clients with newly diagnosed HIV+, out of care HIV+, sexually transmitted diseases, and clients interested in PrEP and/or for follow-up care for PEP.
  • Provide counseling and PrEP/PEP education for HIV- clients.
  • Document patient interactions and communications in the EMR per program protocols.
  • Participate in focus groups and individual interviewing of patients for program enhancement.
  • Attend job-related education, training, seminars, and conferences as determined by the program manager.
