The Patient Care Manager coordinates each patient’s progression of care, ensuring services are patient-centered, high-quality, evidence-based, appropriate, efficient, and cost-effective. The role requires strong priority-setting and time-management abilities, comprehensive knowledge of the Case Management Society of America’s standards of practice, and eligibility to sit for and successfully complete the Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification examination within two years of employment. Success also depends on strong communication, interpersonal, leadership, and negotiation skills, as well as the ability to inspire collaboration and deliver outstanding patient outcomes. Candidates may qualify as registered nurses or social workers. Registered nurse qualifications include current Michigan RN licensure and at least two years of progressive clinical experience. A Bachelor of Science in Nursing is preferred; two years of relevant nursing experience may be considered in lieu of a bachelor’s degree in nursing. Social worker candidates must have an MSW and a minimum LLMSW limited license at the time of hire.
