About The Position
The Patient Care Coordinator (PCC) acts as a liaison between prospects, patients, AltaMed, and the community it serves. The role provides a channel through which patients can seek solutions to problems, concerns, and unmet needs. The PCC advocates for patients and their families on non-medical issues and assists with concerns affecting their overall experience at AltaMed, including quality of care, staff interactions, access to care, enrollment assistance, general assistance, and information.
Requirements
- Minimum high school diploma.
- Certified Application Assessor (CAC) certification is preferred or must be obtained within 90 days of employment.
- Bilingual ability in Spanish and English, English and Mandarin Chinese, English and Cantonese, or English and Vietnamese is strongly preferred.
Nice To Haves
- Bachelor of Science or Bachelor of Arts degree in healthcare or business administration, or a related field of study.
- At least 3 years of direct outreach, marketing, or sales experience in the healthcare industry.
- At least 3 years of customer service experience.
- Experience working with underserved populations.
- Certified Application Assessor (CAC) certification.
