The Patient Care Coordinator (PCC) is a Registered Nurse with a bachelor's degree in nursing who educates patients about their health conditions and develops care plans to address their discharge needs. The PCC facilitates communication among patients, families, caregivers, social service organizations, and healthcare providers, and coordinates care and resources to support safe returns home and achieve quality and desired patient outcomes. The role partners with unit teammates and leaders to support effective patient flow and capacity, reduce length of stay, and decrease readmissions. The PCC assesses, plans, implements, and directs patient care coordination across the continuum. The role includes coordinating discharge plans, improving outcomes and patient experience, preventing errors and hospital-acquired conditions, supporting patient and family engagement, and promoting evidence-based, patient-centered care. The PCC also teaches, coaches, and mentors staff, patients, and families about discharge planning, home resources, quality care, and improved outcomes, and advocates for effective interdisciplinary collaboration. The position involves frequent interaction with people inside and outside the organization. Work requires judgment in solving complex problems and coordination with other departments. The work environment includes considerable exposure to occupational risks and potential bloodborne pathogens and other infectious or biohazardous materials. The role requires moderate physical effort, including handling lightweight materials for about half the day and occasional handling of medium-weight objects.
