About The Position

The Patient Care Coordinator develops and maintains relationships with hospital, skilled nursing, and assisted living facility discharge planning services and/or management, providing ongoing updates about the Company's services available in the market. The role conducts facility visits at physicians' requests to help the program's clinical team determine patient eligibility, collaborates with program staff and field sales resources, participates in weekly program business development meetings, and assists with processing physician orders. The position reports to a Market Executive of Sales.

This patient-facing role is part of Humana's Tuberculosis (TB) screening program; selected candidates must undergo TB screening. The role requires travel within an assigned territory and to sales meetings as required. Scheduled weekly hours are 40.

Requirements

• An RN or LPN license is required.
• Knowledge of governmental regulations and Medicare eligibility requirements.
• Comprehensive understanding of potential patient care plan needs and coordination of necessary resources.
• Excellent customer service, relationship-building, organization, and time-management skills.
• Must undergo TB screening if selected for this patient-facing role.

Nice To Haves

• Three years of clinical experience is preferred.
• A BS in Nursing is preferred.
