About The Position

The Patient Care Coordinator Nurse develops and maintains relationships with hospital, skilled nursing, and assisted living facility discharge-planning services and management, providing updates on the company’s services available in the market. The role includes visiting facilities at physicians’ request to help the program’s clinical team determine patient eligibility, collaborating with program staff and field sales resources, participating in weekly business development meetings, and assisting with processing physician orders. This patient-facing role requires travel within an assigned territory and to sales meetings as needed, and participation in the company’s tuberculosis screening program.

Requirements

RN or LPN license required.
Knowledge of governmental regulations and Medicare eligibility requirements.
Comprehensive understanding of potential patient care plan needs and coordination of necessary resources.
Excellent customer service, relationship-building, organization, and time management skills.
Must be screened for tuberculosis if selected for the patient-facing role.

Nice To Haves

Three years of clinical experience preferred.
Bachelor of Science in Nursing preferred.

Responsibilities
