About The Position

The Patient Care Coordinator (RN) helps patients transition from an inpatient setting back home or to a post-acute setting. The role develops, initiates, and monitors plans of care that support patient self-management, using motivational interviewing techniques to enhance outcomes. The coordinator assesses and coordinates options and services to meet patients’ needs, identifies social determinants of health and refers patients to community resources and social worker support, documents accurately in the medical record and CareTrac, and assists providers with closing care gaps through patient outreach.

Requirements

• Graduate of an accredited school of nursing as a registered nurse.
• At least two years of experience.
• Current state nursing license in good standing.

Responsibilities
