Foundations Recovery Network (FRN) is the premier provider of integrated treatment for co-occurring addiction and mental health concerns, offering residential and outpatient services nationwide. Requirements

Bachelor’s degree in psychology, social work, or related field and or the educational and experience equivelent.
2 years’ experience in a mental health/healthcare field preferred.

Responsibilities

Cordially and professionally meet with and provide after care direction/resources for patients.
Promptly responds to calls/mail in a professional, courteous, and helpful manner.
Maintain filing system for patient after care needs and possibilities.
With the assistance of the clinical care team and the patient; formulate Aftercare Plans that will ensure best possible outcomes
Completes the Referral and Intake Processes accurately as it to pertains to referrals to aftercare referral source
Meet with business development representatives and referral sources to ensure PCC has resources across the country that meet patient needs
Observes all confidentiality regulations.
