About The Position

The Patient Care Coordinator position with the Department of Human Services offers a chance to make a strong impact on patient well-being at Warren State Hospital. You will support important decisions that help ensure safe, timely, and appropriate patient care. This role provides meaningful work in a supportive environment where teamwork and clear communication matter every day. Your efforts will help improve the quality of care throughout the hospital. This position supports the review and evaluation of patient records to ensure that admissions and continued stays meet medical and regulatory standards. The role requires careful study of rules, coordination with hospital teams, and consistent review of patient information.

Requirements

  • One year as a Psychiatric Nurse 2 in a state hospital or a Nurse 2 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) in a State General Hospital or Restoration Center; or Three years of professional psychiatric nursing or general nursing, including one year at an advanced level in a state hospital, general hospital or Restoration Center; and graduation from an approved school of professional nursing; or Two years of professional social or rehabilitative work, including one year of clinical experience in a supervisory, administrative or consultative capacity in a state hospital, state general hospital or Restoration Center; and a bachelor's degree with major course work in social work, nursing, health care administration, behavioral sciences or related fields; or Any equivalent combination of experience and training.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance (conditional offer of employment requires submission and approval).
  • Medical examination and drug screening (conditional offer of employment requires).

Responsibilities

  • Review patient records to determine the medical need for admission and continued stay
  • Track and evaluate patient length of stay and ensure timely review of extended stays
  • Analyze documentation to support compliance with state and federal requirements
  • Work with hospital staff to support communication, treatment planning, and documentation needs
  • Prepare summaries and reports for committees, leadership, and program monitoring activities
