About The Position

Avenues Home Care is hiring a Patient Care Assistant (PCA) to provide care for elderly, vulnerable, and disabled clients, including people who cannot care for themselves. The PCA is expected to prioritize clients’ needs and deliver care with attention, honesty, accountability, and integrity. The Chattanooga office serves the surrounding local area, including Jasper, Cleveland, Signal Mountain, Dayton, Ooltewah, Hixson, Dalton and Lookout Mountain in Georgia, and Ringgold. Care is provided according to each client’s intake plan of care. The PCA reports daily activities to a supervisor through the agency management system and daily activity logs, using other available communication methods as needed. The role also includes maintaining confidentiality, following company policies and procedures, attending requested meetings and in-services, and reporting suspected abuse or neglect and potential changes in a client’s condition. The posting lists full-time, part-time, PRN, and short-shift work, with 8-, 10-, and 12-hour shifts available. The employment type for a particular opening is not specified.

Requirements

Must be at least 18 years old.
Must maintain an established work schedule.
Must have good communication skills and the ability to follow verbal and written assignments.
Must have effective interpersonal and communication skills, including tact and diplomacy.
Must have effective organizational and planning skills, attention to detail, and follow-through.
Must be able to establish and maintain effective working relationships.
Must maintain confidentiality of work-related information and materials.
Must maintain a fit-for-duty physical condition.
Must have a valid driver’s license and automobile insurance.
Must be able to lift or push up to 50 pounds.
Must possess the required skills and be able to perform the essential functions of the job, with or without reasonable accommodations.
Must demonstrate proficiency with the Homecare Company Management System, ClearCare.

Nice To Haves

CNA license or alternate experience as a caregiver.
CPR and First Aid certifications.

Responsibilities

Assist clients with Activities of Daily Living (ADLs) specified in their plans of care, including bathing, dressing, grooming, personal hygiene, toileting, transferring, eating, and continence care.
Provide medication assistance, housekeeping, laundry, meal preparation, feeding, shopping, transportation to and from appointments, and companionship as specified in the client’s plan of care.
Complete other household tasks specified in the client’s plan of care.
Report daily activity to a supervisor through the agency management system and daily activity logs, and communicate by phone, email, agency app, or face to face as appropriate.
Practice safety in all activities involving clients and report unsafe working conditions or behavior.
Provide superior care and remain alert to potential care concerns or changes in a client’s condition.
Report suspected abuse or neglect immediately through the proper chain of command, as required by law.
Document unusual behavior, events, conditions, or people.
Maintain confidentiality of records, correspondence, and other information, following company policies and HIPAA-related procedures.
Maintain personal appearance in compliance with company policies, including the dress code.
Arrive on time for shifts and report scheduling changes promptly.
Attend meetings and in-services as requested, follow company policies and procedures, and perform other duties as assigned.
