About The Position

Mountain Valley Hospice & Palliative Care is hiring an LPN or RN to serve as a liaison between its palliative care practice and skilled nursing facilities (SNFs) and assisted living facilities (ALFs) in Forsyth, Stokes, and Surry Counties, North Carolina. The role focuses on strengthening communication, coordinating care, and supporting patients and families facing serious illness. The position is scheduled Monday through Friday, 8:00 AM to 5:00 PM, and involves travel throughout the service area.

Requirements

• Current LPN or RN license in North Carolina.
• Nursing experience required.
• Excellent communication, organizational, and relationship-building skills.
• Ability to work independently and collaborate with facility staff and healthcare providers.
• Valid driver's license and willingness to travel throughout Forsyth, Stokes, and Surry Counties.

Nice To Haves

• Palliative care, hospice, or long-term care experience preferred.

Responsibilities

• Serve as a liaison between Mountain Valley's palliative care practice and assigned SNFs and ALFs.
• Build positive relationships with facility nursing staff, administrators, and care teams.
• Facilitate communication regarding patient needs, changes in condition, and care coordination.
• Educate facility staff, patients, and families about palliative care services.
• Assist with coordinating patient evaluations, appointments, and follow-up needs.
• Identify patient concerns and communicate with appropriate clinical team members.
• Maintain accurate documentation of patient interactions and care coordination activities.
