About The Position

The PACE Center Manager is responsible for a wide range of disciplines and services provided through the FHCN PACE Center, including all administrative duties related to the direct operation of the FHCN PACE Center. The Center Manager coordinates the overall operations of the FHCN PACE Center and serves as the liaison between direct care staff and administration.

Requirements

  • At least 2 years of experience in a supervisory capacity within a healthcare setting.
  • Experience with senior care clinical programs similar to FHCN PACE preferred.
  • Either have one year of experience working with a frail or elderly population or, in the absence of such expertise, receive appropriate training from the FHCN PACE on working with a frail or elderly population upon hiring.
  • Be medically cleared for communicable diseases and have all immunization dates before engaging in direct participant contact.
  • Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.
  • Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs, and other elements.
  • Ability to use Microsoft Excel to analyze data, including the use of formulas, functions, lookup tables, and other standard spreadsheet elements.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.
  • Meet a standardized set of competencies for the specific position description established by FHCN PACE and approved by regulatory agencies before working independently.
  • BLS certified.
  • Job duties require the employee to effectively communicate, verbally and in writing, their opinions and extrapolations of information they collect and synthesize/analyze.
  • Responsible for resolving conflicts arising from disagreements between employees, between employees and customers/clients, or with the public, other legal entities, or governmental authorities.
  • Compiles, analyzes, and prepares information in effective written form, including correspondence, reports, articles, or other documentation.
  • Effectively conveys technical information to non-technical audiences.

Nice To Haves

  • Bachelor’s Degree in a health-related field preferred, not required.
  • Registered Nurse license is preferred but not required.

Responsibilities

  • Provides management to departmental staff.
  • Responsible for performance management of assigned supervisors, and those supervisors’ assigned staff, including recognition, performance evaluations, formal coaching and counseling, and making decisions or recommendations regarding necessary disciplinary actions.
  • Responsible for making recommendations regarding hiring or firing, and the advancement and promotion of assigned supervisors, and those supervisors’ assigned staff, or any other change in status of assigned supervisors, and those supervisors’ assigned staff.

  • Ensures department employees receive instruction/training that complies with the training plan, including on-the-job training to develop department employees and supervisors.
  • Works with the supervisor to ensure necessary remediation is taken.
  • Recommends workflow, policy, and procedure changes based on observations from performance metrics, outcomes, and feedback from the supervisor.
  • Ensures the department maintains compliance with all employee-related reporting and tracking.
  • Coordinates all center activities, including care planning and implementation, preparation for interdisciplinary conferences and daily meetings, family conferences, and initial and periodic assessments.
  • Responsible for registered nurses, rehabilitation therapists, clerical staff, social workers, transportation staff, recreational staff, registered dietitian, and center aides.
  • Facilitates and leads morning meetings of the interdisciplinary team (IDT); facilitates and leads regularly scheduled Intake and Assessment meetings to review participant cases regularly.
  • Participants as a member of the IDT.
  • Works closely with the medical care team as part of the IDT to develop and implement participants’ individualized care plans.
  • Oversees the quality of care and safety of FHCN PACE participants, both at the FHCN PACE center and in the community.
  • Responsible for overseeing various functions of the PACE center operations.
  • Ensures day center activities and services are well-planned in advance and organized.
  • Ensure the health center maintains compliance with DHCS and CMS requirements, including daily maintenance of clinical logs (quality controls, crash cart, etc).
  • Manages health center operations and resources to ensure goals are met.
  • Conducts regular inspections of work areas and exam rooms to ensure compliance with OSHA, Joint Commission, and other regulatory guidelines.
  • Ensures internal audits are completed to ensure compliance.
  • Ensures all CMS requirements for PACE onboarding is completed.
  • Responsible for coordinating and participating in the Annual Skills Proficiency/Competency tests.
  • Serves as a liaison between disciplines, administration, and direct staff.
  • Establishes and maintains effective interpersonal relationships with all levels of staff, other departments, agencies, members, providers, and the general public.
  • Coordinates 24-hour care delivery.
  • Coordinates with the enrollment/outreach team to hold “friendly visits” for new candidates at the FHCN PACE center.
  • Serves as a representative of FHCN PACE to the community.
  • Coordinates activities related to new participants.
  • Manages the center’s budget; communicates regularly with FHCN PACE administration on center operations and needs.
  • Oversees overall documentation cohesiveness and accuracy in each participant’s medical record.
  • Other projects and duties as assigned.
