About The Position

The Manager, PACE Operations is responsible for all disciplines and services provided through the Prisma Health Senior Care center as part of the Program of All-inclusive Care for the Elderly (PACE), including all administrative responsibilities related to the direct operation of the center. Requirements

  • Bachelor's degree in Nursing
  • Four (4) years of experience in the field of nursing, social service, sociology, psychology, or in an area closely related to health and social development for the aging.
  • One (1) year of working with the frail or elderly population.
  • Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
  • BLS certification
  • Knowledge of office equipment (fax/copier)
  • Proficient computer skills (word processing, spreadsheets, database, data entry)
  • Mathematical skills
  • Clerical testing skills

Nice To Haves

  • Experience working in a managed health care environment with elements of care coordination and at-risk reimbursement preferred.

Responsibilities

  • Coordinates the delivery of health care in the home and in the PACE center through the supervision of staff responsible for ensuring participant needs are met 24 hours per day, seven days per week.
  • Coordinates all center activities including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences and all related follow up.
  • Directly and indirectly supervises center staff, including nurses, therapists, social workers, transportation, and administrative staff.
  • Responsible for hiring, performance review, and other related human resource duties.
  • Operates the center within established operating and capital budgets.
  • Monitors financial reports for variances and provides explanations with plans of action to leadership.
  • Participates in the development of annual budgets with the Director, PACE Operations.
  • Documents participant changes appropriately in the medical record and communicates participant changes to team members.
  • Serves as liaison between individual program disciplines, and community service providers.
  • Leads team meetings daily and participates in PACE Leadership team monthly and serves as a liaison between administration and direct staff.
  • Performs other duties as assigned.
