About The Position

The PACE Center Manager is responsible for a wide range of disciplines and services provided through the FHCN PACE Center, including all administrative duties related to the direct operation of the FHCN PACE Center. The Center Manager coordinates the overall operations of the FHCN PACE Center and serves as the liaison between direct care staff and administration.

Requirements

  • At least 2 years of experience in a supervisory capacity within a healthcare setting.
  • Experience with senior care clinical programs similar to FHCN PACE preferred.
  • Either have one year of experience working with a frail or elderly population or, in the absence of such expertise, receive appropriate training from the FHCN PACE on working with a frail or elderly population upon hiring.
  • Be medically cleared for communicable diseases and have all immunization dates before engaging in direct participant contact.
  • Pass a criminal background check verifying no exclusion from Medicare/Medicaid programs, no convictions related to health care program fraud, and no convictions of abuse.
  • Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs, and other elements.
  • Ability to use Microsoft Excel to analyze data, including the use of formulas, functions, lookup tables, and other standard spreadsheet elements.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.
  • Meet a standardized set of competencies for the specific position description established by FHCN PACE and approved by regulatory agencies before working independently.
  • BLS certified.
  • Job duties require the employee to effectively communicate, verbally and in writing, their opinions and extrapolations of information they collect and synthesize/analyze.
  • Responsible for resolving conflicts arising from disagreements between employees, between employees and customers/clients, or with the public, other legal entities, or governmental authorities.
  • Compiles, analyzes, and prepares information in effective written form, including correspondence, reports, articles, or other documentation.
  • Effectively conveys technical information to non-technical audiences.

Responsibilities

  • Provides management to departmental staff, including performance management, hiring/firing recommendations, fostering a culture of accountability, developing action plans, empowering staff, delegating duties, monitoring departmental budgets, ensuring regulatory compliance, overseeing training, recommending policy changes, and tracking employee-related reporting.
  • Coordinates all center activities, including care planning and implementation, preparation for interdisciplinary conferences and daily meetings, family conferences, and initial and periodic assessments.
  • Responsible for managing registered nurses, rehabilitation therapists, clerical staff, social workers, transportation staff, recreational staff, registered dietitians, and center aides.
  • Facilitates and leads morning meetings of the interdisciplinary team (IDT) and regularly scheduled Intake and Assessment meetings.
  • Participates as a member of the IDT, working closely with the medical care team to develop and implement participants’ individualized care plans.
  • Oversees the quality of care and safety of FHCN PACE participants, both at the FHCN PACE center and in the community.
  • Oversees various functions of the PACE center operations, including ensuring day center activities are well-planned, maintaining compliance with DHCS and CMS requirements, managing health center operations and resources, conducting regular inspections for compliance with OSHA and Joint Commission guidelines, ensuring internal audits are completed, ensuring CMS requirements for PACE onboarding are met, and coordinating Annual Skills Proficiency/Competency tests.
  • Serves as a liaison between disciplines, administration, and direct staff, establishing and maintaining effective interpersonal relationships.
  • Coordinates 24-hour care delivery.
  • Coordinates with the enrollment/outreach team to hold “friendly visits” for new candidates.
  • Serves as a representative of FHCN PACE to the community.
  • Coordinates activities related to new participants.
  • Manages the center’s budget and communicates regularly with FHCN PACE administration on center operations and needs.
  • Oversees overall documentation cohesiveness and accuracy in each participant’s medical record.
  • Other projects and duties as assigned.
