About The Position

The Outpatient Navigator facilitates inpatient discharges to the next level of care with a focus on transitioning care from inpatient setting to The Brook Hospitals outpatient programs. The Outpatient Navigator provides groups and individual education to patients on the importance of continuing care. The Outpatient Navigator will be responsible for communicating eligibility to key members of senior leadership and associated departments and will be responsible for routinely collecting and entering data, specific to outpatient eligibility, into Midas for trending report purposes. The Outpatient Navigator will have a thorough understanding of the Step-Down playbook , implementing components of the playbook as needed. A thorough understanding of the inpatient workflow process, as it pertains to case management and discharge planning is preferred. Otherjob functions may include: obtaining psycho-social assessments; conducting family/group therapy on assigned cases; complete documentation per hospital policy and procedure; case coordination; coordinate discharge planning; assist in developing and maintaining community resource information; performs marketing functions as assigned; assists in the maintenance of the program environment in compliance with policies of safety and security; ensures good clinical judgment to ensure optimum patient care; participate in continuing education activities.

Requirements

  • Master's Degree from an accredited school in the Behavioral Sciences
  • Must be licensed or under supervision in state of Kentucky (CSW/LCSW CADC/LCADC, MFTA, LMFT, LPCA, LPCC, LPA, LPP)

  • One to three years experience
  • A thorough understanding of the inpatient workflow process, as it pertains to case management and discharge planning is preferred.
