About The Position

Virginia Mason Franciscan Health Rehabilitation Hospital, operated jointly with Lifepoint Health and Franciscan Health, is seeking an Outcomes Manager (PRN) to join their team. This role is crucial in ensuring the timely and accurate completion of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) for each rehabilitation inpatient. The Outcomes Manager will collect, review, and monitor information within the IRF-PAI to ensure accurate scoring of functional status and reflection of patient care needs, as documented in the medical record and the UDS Pro-Doc system, in collaboration with the interdisciplinary team. This position also assists in the ongoing monitoring of identified outcome benchmarks for the inpatient population and requires maintaining the confidentiality of all patient information.

Requirements

  • Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program or Licensed/Registered PT, OT, SLP from an accredited school.
  • Current state licensures such as Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SLP).
  • CPR/BCLS certification.

Nice To Haves

  • 3 to 5 years of experience in the care of patients in the acute hospital, skilled nursing facility or rehabilitation setting.
  • Excellent oral and written communication and interpersonal skills.
  • Ability to work independently.
  • Flexibility with working up to 20 hours per week, including some weekends.

Responsibilities

  • Provides coordination of the timely and accurate data completion of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) for each rehabilitation inpatient.
  • Collects, reviews and monitors the information contained in the IRF-PAI, on each patient to assure accuracy in the scoring of functional status and reflection status and reflection of the patient’s care needs, as documented in the medical record and the UDS Pro-Doc system, in collaboration with the interdisciplinary team.
  • Assists in the ongoing monitoring of identified outcome benchmarks for the inpatient population.
  • Access to confidential patient information and maintains all confidentiality of all information.
