About The Position

Houston Hospice is seeking compassionate On-Call Registered Nurses to support patients and families during evening and overnight hours. The role provides after-hours clinical guidance, in-home nursing visits, symptom management, comfort-focused care, and support with end-of-life needs. Nurses collaborate with physicians and interdisciplinary team members and document care in the electronic medical record. Additional orientation may be available for qualified nurses transitioning into hospice care.

Requirements

• Graduate of an accredited nursing program.
• Current Texas RN license.
• Current Basic Life Support (BLS) certification.
• Strong clinical assessment, communication, and critical-thinking skills.
• Ability to work independently and respond confidently to changing patient needs.
• Reliable transportation and willingness to travel to patients' homes.

Nice To Haves

• Hospice, palliative care, home health, or hospital nursing experience.
• Experience with electronic medical records.

Responsibilities

• Respond to after-hours patient and family calls with clinical guidance, reassurance, and support.
• Conduct in-home nursing visits to assess patient needs, manage symptoms, and provide comfort-focused care.
• Educate and support patients and caregivers through changes in condition and end-of-life needs.
• Collaborate with physicians and interdisciplinary team members to coordinate patient care.
• Accurately document assessments, interventions, and patient updates in the electronic medical record.
