On-Call RN
About The Position
Hearth Hospice is seeking a full-time On-Call Registered Nurse to provide care and support to hospice patients and their families. The role identifies patient and family needs, coordinates care, and provides preventative measures and compassionate support during critical moments. The territory is Dayton. The schedule is 7 on/7 off: Friday at 4:30 p.m. through Monday at 8 a.m., and Monday through Thursday at 4:30 p.m. through 8 a.m. Hearth Hospice describes itself as a mission-driven, patient-centered leader in end-of-life care. The organization aims to provide patients and loved ones with support, comfort, compassion, dignity, peace, and human connection.
Requirements
- Current, unencumbered Registered Nurse (RN) license.
- At least one year of RN experience in a medical-surgical or acute-care setting.
- Computer proficiency, including typing and use of various programs; familiarity with electronic medical records (EMR).
- Current CPR certification if required by the state.
Nice To Haves
- Bachelor’s degree in nursing or health care preferred.
- Hospice experience is a plus.
Responsibilities
- Respond to patient needs ranging from urgent to routine.
- Conduct comprehensive assessments of patients’ physical, emotional, social, and spiritual needs.
- Maintain accurate documentation in patients’ homes and in accordance with agency and regulatory standards.
- Educate, guide, and support families in caring for their terminally ill loved ones.
