About The Position

Visiting Nurse Association of Southeast Missouri is seeking an Occupational Therapist to join its VNA Home Health team. The nonprofit Home Health program provides physician-ordered skilled services in patients’ private homes.

The Occupational Therapist provides skilled therapy to referred patients and follows the physician’s plan of treatment when planning and administering care. The role includes supporting functional progress toward therapy goals, communicating relevant changes in a patient’s condition to the TCM and Case Manager, consulting with other team members, educating staff and patients and families, arranging physician-ordered equipment to improve function and independence, and evaluating discharge planning needs.

VNA provides new hires with 30 days of preceptor and orientation training. Training includes shadowing Home Health team members, accompanied patient visits, and instruction in using the handheld device for patient visits. Additional responsibilities and qualifications will be discussed during the interview.

Requirements

• Degree earned from a program approved by the American Occupational Therapy Association.

Nice To Haves

• At least one year of experience as a licensed occupational therapist in a health care institution is preferred.

Responsibilities

• Provide skilled occupational therapy services to referred patients and clients.
• Follow the physician’s plan of treatment when planning and administering therapy.
• Assist with identifying problems and progressing toward occupational therapy procedure goals and functional activities based on the patient’s evaluation.
• Communicate changes in a patient’s condition that warrant a revision to the plan of care to the TCM and Case Manager.
• Consult with other team members.
• Provide education to staff, patients, and families.
• Provide or order physician-ordered equipment to increase patient function and independence.
• Evaluate discharge planning needs.
