About The Position

Health Link Home Health is hiring an Occupational Therapist to provide home health services in an assigned Bay Area territory. The role offers hourly pay for scheduled hours, including drive time and documentation, with no per-visit pay or unpaid charting. The Santa Clara County position is full-time; the posting also mentions full-time or part-time schedules in San Francisco or San Mateo County. Territories and scheduling are designed to minimize travel time and support a manageable caseload. The Occupational Therapist evaluates patients, develops and carries out individualized plans of care, supports functional independence and safety at home, educates patients and caregivers, coordinates with the interdisciplinary team, and completes documentation. Compensation is $60–$75 per hour based on experience, with a $10,000 sign-on bonus and productivity bonuses of up to 25% of wages. This is a direct-hire position, not a contract role.

Requirements

• Current California occupational therapist (OT) license.
• Valid California driver's license.
• Ability to travel regularly within the assigned territory.

Nice To Haves

• Home health experience is preferred.
• Strong outpatient, skilled nursing facility (SNF), or hospital experience is welcomed; home health orientation and OASIS training will be provided.

Responsibilities

• Complete evaluations, re-evaluations, and discharges, and develop individualized plans of care based on physician orders.
• Provide skilled treatment focused on activities of daily living (ADLs), adaptive equipment, home safety, and functional independence.
• Educate patients and caregivers on safe, independent routines at home.
• Coordinate with physicians, case managers, and the interdisciplinary team.
• Complete documentation using AI-assisted tools.
• Travel regularly within the assigned territory.
