About The Position
We are seeking a dedicated Occupational Therapist to join the Home Health team. The role provides high-quality therapy services in patients’ homes and supports them in achieving their rehabilitation goals. The therapist typically averages four home visits per day and works independently in a home environment. The coverage area includes northeastern Sacramento into Folsom and Woodland, with services in Yolo County. Mileage is reimbursed at the GSA rate with prior approval. The therapist will be the first to float as required. COTAs with home health experience may be considered if no OT applicants are available.
Requirements
- At least 2 years of experience in a healthcare setting.
- Current Occupational Therapy license in California.
- BLS Certification.
- National Provider Identifier (NPI) number.
- Valid driver’s license and proof of automobile insurance.
- Strong clinical skills, a compassionate approach to patient care, and the ability to work independently in a home environment.
Nice To Haves
- Home health experience for COTAs; COTAs may be considered if no OT applicants are available.
Responsibilities
- Conduct patient evaluations and develop customized treatment plans.
- Provide therapy sessions in patients’ homes, typically averaging four visits per day.
- Collaborate with other healthcare professionals to provide comprehensive care.
- Document patient progress and adjust treatment plans as necessary.
- Educate patients and their families on recovery strategies and exercises.
- Float as required.
