About The Position

One Home Health is a full-service, Medicare-certified home health agency licensed in Illinois and Indiana. Our mission is to deliver compassionate, personalized in-home care through a highly trained, world-class team committed to exceptional clinical outcomes and patient satisfaction. We are seeking a part-time licensed Occupational Therapist (OT) to join our exceptional team in Indiana. We have openings in Lake County (Crown Point, Hobart, Gary, Merrillville, Munster, etc.), Porter County (Valparaiso, Portage, Porter, Boone Grove, South Haven, etc.), and the surrounding areas.

Requirements

• Active licensure as an Occupational Therapist (OT) in good standing
• Demonstrated ability to work independently while effectively managing a flexible schedule
• Reliable, professional, and committed to delivering patient-centered care

Nice To Haves

• Prior home health experience preferred; new candidates with strong clinical skills encouraged to apply
• Excellent communication and clinical documentation abilities

Responsibilities

• Conduct comprehensive in-home occupational therapy evaluations and deliver targeted treatment interventions
• Develop and implement individualized care plans designed to maximize patient independence and functional outcomes
• Educate patients and caregivers on safety practices, adaptive techniques, and proper use of assistive equipment
• Complete accurate, timely, and compliant clinical documentation for all patient visits
• Collaborate effectively with the interdisciplinary team to communicate patient progress and adjust care plans as needed
• Ensure adherence to all clinical, regulatory, and documentation standards
