About The Position

FOX Rehabilitation is hiring licensed Occupational Therapists to provide one-on-one care to older adults in Jonesboro, Georgia, and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians may also incorporate telehealth and other virtual care into their schedules.

The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and support patients’ mobility, independence, wellness, longevity, and quality of life. FOX describes itself as a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism, with a collaborative interdisciplinary team and clinician-driven culture. Multiple employment arrangements are available, including full-time, part-time, and PRN/FOX Flex. New graduates may be eligible for the Emerging Professionals Mentor Program.

Requirements

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; assisting with patient transfers and bed mobility; repositioning patients; standing and walking for extended periods; climbing stairs; and performing therapeutic activities.
• Ability to drive to patient homes and senior living communities.
• Ability to wear PPE when required.

Responsibilities

• Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy that supports patient wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Create and manage a treatment schedule and build patient relationships.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
