About The Position

Trinity Health At Home is hiring a Home Care Occupational Therapist to provide compassionate, high-quality therapy services to clients in their homes. The role helps individuals regain independence and improve their quality of life through personalized care plans and therapeutic interventions. Coverage regions include Westland, Garden City, Wayne, Dearborn, Dearborn Heights, and Taylor.

The Occupational Therapist assesses clients’ functional status, develops individualized treatment plans in collaboration with physicians, and provides preventive, therapeutic, and rehabilitative services according to each plan of care. The role includes educating clients and families, supervising Certified Occupational Therapy Assistants and Home Health Aides, coordinating with interdisciplinary teams, participating in case conferences, and documenting care using point-of-care systems.

Requirements

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification and state licensure or registration.
• At least one year of experience or completion of a 6–8 week preceptorship.
• Valid driver’s license and reliable transportation.
• Commitment to ethical standards and continuous learning.

Nice To Haves

• Home health experience preferred.

Responsibilities

• Assess clients’ functional status and develop individualized treatment plans in collaboration with physicians.
• Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.
• Educate clients and families on safety, health promotion, and use of prosthetic and orthotic devices.
• Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).
• Coordinate care with interdisciplinary teams and participate in case conferences.
• Document all care activities accurately using point-of-care systems.
