About The Position
Human Touch Home Health is actively hiring Occupational Therapists (OTs) for Full-Time, Part-Time, and PRN home health opportunities throughout Upper Marlboro and Southern Prince George’s County. Our patients want to do more than recover. They want to get back to living. Getting dressed without assistance. Making something to eat. Getting safely in and out of the shower. Moving through their home without fear. Doing the everyday things they once did without thinking twice. That's where you come in.
Requirements
- Active Maryland Occupational Therapist License Required
- Graduate of an accredited Occupational Therapy program
- Reliable transportation
- Strong clinical judgment and communication skills
- Ability to work independently in patients’ homes
- Excellent organizational and documentation skills
Nice To Haves
- Home Health Experience Preferred, but Not Required
- WellSky Experience is a Plus
Responsibilities
- Perform comprehensive OT evaluations
- Develop and update individualized plans of care
- Assess functional limitations and barriers to independence
- Address activities of daily living, including bathing, dressing, grooming, toileting, and meal preparation
- Provide therapeutic activities to improve functional performance
- Assess home safety and fall risks
- Recommend adaptive equipment and modifications
- Provide training in safe transfers and functional mobility
- Develop home programs based on patient goals
- Educate patients, families, and caregivers
- Monitor progress and adjust treatment plans as appropriate
- Complete reassessments and discharge planning
- Collaborate with the interdisciplinary home health team
- Complete accurate and timely documentation using WellSky
