Occupational Therapist- Plainview, NY
About The Position
FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in Plainview, NY, and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care, through FOX’s Geriatric House Calls™ model. Clinicians may also provide telehealth and other virtual care when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and support patients’ mobility, independence, wellness, and quality of life. Clinicians work in a collaborative, interdisciplinary environment and may have opportunities to increase income through referrals, clinical instruction, mentoring, supervision, care above quota, and content creation. Full-time, part-time, and PRN/FOX Flex employment pathways are described. The New York standard base pay range for a full-time role is $70,000–$105,000 annually; part-time and PRN pay is prorated. Benefits are available for eligible roles.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; providing moderate to maximum assistance with transfers and bed mobility; repositioning patients; standing and walking for extended periods; and climbing stairs.
- Ability to drive to patient homes and senior living communities.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Perform CPR and assist with patient transfers, bed mobility, repositioning, and therapeutic activities.
- Wear personal protective equipment when required.
