Occupational Therapist – Home Health Riverside Home Health & Hospice Southern Oregon | Medford & Grants Pass Full-Time | Part-Time | PRN $50–$55/hour Help Patients Get Back to Living Life Occupational therapy in home health is different. Why This Role Matters Our Occupational Therapists help patients regain independence after illness, injury, or hospitalization. In this role you will: • Evaluate patient functional abilities and daily living skills • Develop individualized treatment plans to improve independence • Teach patients adaptive strategies and new ways to complete daily activities • Identify home safety risks and recommend modifications or equipment • Educate patients and caregivers on safe mobility and self-care • Collaborate with physicians, nurses, and therapy team members Your work helps patients remain safely in their homes and return to the activities that give life meaning. What You’ll Be Doing • Evaluating patient functional status and independence with daily activities • Assessing home environments for safety risks and barriers • Developing individualized treatment plans and therapy goals • Teaching adaptive techniques for daily living skills • Recommending and training patients on assistive equipment • Educating caregivers and family members • Supervising Certified Occupational Therapy Assistants when appropriate • Collaborating with the interdisciplinary home health team Why Therapists Choose Home Health Many therapists find home health allows them to practice therapy the way it was meant to be practiced.
