About The Position

Join our team and grow with us both professionally and personally! Acts is currently seeking qualified candidates for our Home Health Occupational Therapist (OT) per-diem position. In this role, you will be responsible for providing occupational therapy services to home health residents/clients in their home environment to promote functional independence, safety, and quality of life. The position involves an in-house program within a welcoming, slower-paced community environment that delivers holistic care with a strong focus on prevention. It offers specialized on-site training delivered during paid work hours, some sessions eligible for CEUs, and access to state-of-the-art rehab gyms and equipment. There is an opportunity to build relationships by following residents across the full continuum of care, including inpatient, home health, outpatient, and fitness services. Clinically driven, patient-centered treatment decisions guide all programs and care plans. Cross training is provided to support all rehabilitation areas within our communities, with limited travel within and between our private communities. As a non-profit organization, student loan forgiveness is available after 10 years.

Requirements

  • Bachelor's degree in Occupational Therapy from an accredited program
  • Valid Occupational Therapist license or eligibility for license in the state(s) where services are rendered

Nice To Haves

  • Minimum of 1-2 years of experience in home health is preferred

Responsibilities

  • Providing occupational therapy services to home health residents/clients in their home environment to promote functional independence, safety, and quality of life
  • Evaluates residents/clients
  • Develops individualized plans of care
  • Implements evidence-based interventions
  • Collaborates with the interdisciplinary care team to achieve optimal outcomes in compliance with physician orders, state practice acts, and federal and state home health regulations
